Allergies and Adverse Reactions Summary

The Allergies and Adverse Reactions Summary page displays a list of all known allergy records that have been extracted from your VA facilities electronic health record.

This Summary page shows a listing of the latest 10 Allergy entries entered by a VA provider. You can re-sort the list by selecting any of the column headers indicated by a link with the up and down arrow symbol up/down button. If you have more than 10 entries, you will see links at the bottom of the table of entries that you can use to index through the entire list. Options include the following:

  • Select the First, Previous, Next, Last or a specific page of entries.
  • Change the default view from 10 entries per page to 25, 50, or 100.

When the Allergies and Adverse Reactions Summary page appears, one of the following messages will display at the top of the page:

  • We are Updating your Allergies and Adverse Reactions Information,
  • Update is Complete, Record Updated,
  • No Records Found,
  • Updates from the following VA electronic medical records are temporarily unavailable, or
  • Updates from the following VA electronic medical records and access to your personal health record are temporarily unavailable.

We are updating your Allergies and Adverse Reactions information - This icon blue warning icon and message appear when the system is retrieving Allergies and Adverse Reactions records from your VA electronic health record. Depending on the number of records being retrieved, this process may take several minutes. You may select the Refresh button to update the page with the most recent Allergies and Adverse Reactions records.

Update is Complete - This icon green warning icon and message appear when the retrieval of your Allergies and Adverse Reactions records from your VA electronic health record is complete.

No Allergy/Adverse Reaction records found - This message appears when no Allergies/Adverse Reactions records are found in your VA electronic medical record.

Updates from the following VA electronic medical records are temporarily unavailable - This icon yellow warning icon and message appear when the system is unable to retrieve your Allergies and Adverse Reactions records from your VA electronic medical records.

Updates from the following VA electronic medical records and access to your personal health record are temporarily unavailable - This icon yellow warning icon and message appear when the system is unable to retrieve your Allergies and Adverse Reactions records or provide access to your personal health records.

Printer Friendly - Select this link to display a printer friendly page designed to print on an 8 1/2 x 11 inch sheet of paper.

Filter Result - This field allows you to select the source of the allergy information to be displayed. Select VA to display all allergies submitted by VA facilities, or select Non VA to display self-entered allergy information. You may also select Both to view a combined listing of VA and self-entered allergy information. Select the Go button to view the returned results.

You cannot add or edit your VA Allergy and Adverse Reaction records. If you use the filter to select Non VA or Both to display self-entered or a combination of VA and self-entered Allergy information, the Add/Edit Self Entered Allergy link will display.

 

Date Entered - This field shows the date the allergen was entered.

Allergen/Reactant - This field shows the names of each item you are allergic to. You may obtain detailed information you entered or your VA provider supplied regarding this item by clicking on the highlighted allergen in this field.

Reaction/Side Effect - This field describes your reaction to each allergen.

Location - This field provides the location of the VA facility that entered this information.

Source - This field reflects the source of this information. If you entered the allergen information, Self will be displayed; otherwise, VA will be displayed.
