Health Information Card
How do I print my health information card?
To print your health information card, you must be registered with a My HealtheVet account.
Once signed in:
Select the Personal Information tab
Select Health Information Card
Select Print
If you are a Veteran and use the VA health care system, you are encouraged to obtain a My HealtheVet Premium account. With this account, you will have access to key portions of your VA health care record (including VA Allergies) and Secure Messaging with your VA health care teams.
Can I edit my health information card?
To edit your health information card, elect Edit to change your address, blood type, and organ donor status.
